MEDICAL FORMS

All new and returning UPS student-athletes must fill out the required documentation prior to any conditioning activities, practices or competition.   If you are having trouble downloading the documents on this page or have any questions, please contact the UPS Athletic Office at (253) 879-3140.

All documents must be returned to the UPS Athletic Training Department by August 1, 2007.   Once they are completed, the forms can be mailed to:
