2008 Flexible Benefits Program

The election guide summarizes the flexible benefits choices provided by the university.  In addition to reading the 2008 election guide, you should:

• Read the September 2008 Benefits Newsletter

• Complete the online health risk appraisal at www.health-eoutlook.com to receive an additional $25.00 per month in flexible benefits allowance dollars (See Health-e Outlook™ brochure and demo for further information).

• Review plan options and complete the Benefits Elections Worksheet
  
  

• Choose one of the three medical plans. (See Health Plan Comparison Sheet FAQ)

  • Choose whether or not to enroll in the dental plan.
  • Reevaluate your life insurance needs.
  • Decide how much to put into personal expense accounts.
    

• Complete the forms in your election guide and return them to Human Resources. In order to receive benefits coverage for calendar year 2008, your forms must be submitted no later than November 30, 2007

Thursday, December 20, 2007: Your election decisions (credits and deductions) will be reflected in your December paycheck for coverage beginning January 1, 2008.
