Student Recital Program Form

At least two weeks (three weeks for vocal recitals) prior to the performance, each performer must submit the requested information below.  Programs will be prepared according to the University Visual Standards Style Guide. 

The program draft will be returned to each performer (and his/her instructor) for final proofing.  The instructor and performer are asked to initial the program, thereby taking full responsibility for the accuracy of spelling, listings, dates, texts, and translations.  Return the signed program draft to Patti Sumich, Office of Public Events, Communications House, 1314 N. Alder Street.

The Office of Public Events will be responsible for the preparation and printing of the programs.

Special Instructions for Vocal Programs
Vocal students are required to include full text and translations for each piece in their program.  Please send this information as an "attachment file" in Microsoft Word format to Patti Sumich (e-mail).

    Acknowledgements should recognize persons who have contributed to your professional development and training.  A brief statement of thanks of a personal nature is acceptable (i.e. family, name of friend).  Statements of personal faith, if included, should simply and respectfully acknowledge thanks.  Acknowledgements are subject to review and revision with the joint consent of the performer and the Office of Public Events.
