REQUESTS AND REPAIRS
Service
Request Form
As
a Residence Life staff member, you will be addressing a number of repair
requests throughout the year. If
the repair is routine (i.e., the
damage will not harm the facility or is not a safety hazard) the student is
responsible for completing the service request request.
When completing service requests, please make sure the following
guidelines are used:

When
filling out the request, it is important that the student be very specific
about the location and nature of the repair work needed.

After the
service request is completed, the form should be signed (at the top of the
form) by the student. The student's signature authorizes facilities services staff
to enter his/her room. Only
indicate a student's name in the billing section if that student is to be
charged and acknowledges that charge.

Your
supervisor should review the form to make sure all information is complete
and accurate. Service request
requests must be turned in by the Resident Director.

Items
such as light bulbs and toilet paper may be requested directly from the hall
custodian.

All
emergency repairs should be called in immediately to the Director of
Residence Life at x3317. If
the work is needed after 5:00 p.m., requests should be made to Security
Services at x3311. The
emergency repair request to Security should only be made if the damage will
harm the facility or if a safety hazard of any kind is present.
Security will determine if an after hours Facilities Services worker
will be dispatched.

All
broken windows should be called in to Security Services for investigation in
addition to filling out a service request.
If this is not done, the students in that room will be charged.

Any time you
suspect that damage may have been intentional, a communication report should
be filed describing the circumstances.
Indicate on the communication report whether or not a service request
has been filed. DO NOT
include the student's name in the billing section unless they assume
responsibility for the damage. Once
a disciplinary investigation has been conducted, we will make sure that the
appropriate individuals are billed for damages.
Simply write "CR" in the billing section of the service
request.
All
Facilities Services Department staff should be wearing the University of Puget
Sound insignia for identification. If
no one is at home at the time the work is done, a notice will be left indicating
that a Facilities Services employee has been there.
Work will be completed between the hours of 9:00 a.m. and 4:30 p.m.,
unless it is an emergency. Facilities Services Department’s intent is to cooperate as
much as possible with students to get the work done as soon as possible.
Your supervisor
is responsible for following up on Service Requests.
Any questions or concerns about uncompleted work should be directed to
this person.
